Music Tech DIY • 4 input AC mixer PCB

I'm looking for a 4 (or more) input AC mixer. AC because I'd like to connect headphones to it.
If working on +/-15V would be even better as it's going into a frac system. (Yes, I could pick some eurorack thing and put some diodes between the power source and the module, but preferably not.)

Any suggestions? Couldn't find anything which was ticking all boxes
